Housing unstable or homeless
Able to perform activities of daily living with support
Committed to housing plan participation
No active substance use requiring clinical detox
How To Refer?
STEP 1
Identify patient who is housing unstable
STEP 2
Complete referral form (online or by phone)
STEP 3
We confirm bed availability within 24 hours
STEP 4
Resident admitted same or next day
